The questionnaire is designed to collect preliminary information. Exact answers are helpful, but an honest “I don’t know” is better than a guess.

- A-Number and USCIS receipt numbers, if available
- Country of citizenship and basic immigration history
- Dates and manner of entry into the United States
- Immigration notices, decisions, or charging documents
- Upcoming court hearings, interviews, appointments, or deadlines
- Prior immigration applications, petitions, orders, or missed hearings
- Complete criminal history, including open cases, if applicable
- Information about a spouse, parent, child, or other relevant relative

Do not include passwords, financial-account details, payment-card information, or original documents.
